The following Forms are applicable in both tryout and non-cut sports and must be printed out, signed and returned before an athlete can participate in any after school sport at Fisher Middle School. These must be turned into Ms. Rynn or Mrs. Mitchell by deadline.
There is a box in main office for turn ins. Please make sure all items are covered below.
Required Forms #1-8:
All athletes MUST fill out an EMERGENCY CONTACT FORM before trying out for any sport every school year. This form must be on file in the PE Office, handed to your athletic directors or turned into the main office in the "after school box" at the registrar's desk.
You must register online with the Los Gatos Recreation Department and pay the after school sports fee. If you have difficulty with registration online, please call Los Gatos Recreation directly, 408-354-8700
Information about symptoms of a concussion and what to do if you think your child has suffered a concussion. Must be filled out for every sport that your child participates in.
A guideline of sportsmanlike behavior for Fisher athletes based on 6 core principles: trustworthiness, respect, responsibility, fairness, caring, and good citizenship. Must be filled out for every sport that your child participates in.

Anyone who plans to drive any students other than their own, MUST have this form on file in the girls PE office with Ms. Rynn/Mrs. Mitchell.

Print out sport specific letter above and use the last page of this letter as your checklist to complete your packet. Place checklist page on top of your completed packet to turn in to Ms. Rynn or Mrs. Mitchell.
