Required Forms

1. The emergency/mandatory documents must be filled out before showing up to tryouts/practice.

No papers need to be delivered to the Athletic Department, only the online "JOT FORM" is necessary to try out/practice.

*If your child makes the team, a separate registration link will be provided once teams are formed (see #2)  

2. Los Gatos Saratoga Rec Department Sign-Up for Fisher After-School Sports

You must register online with the Los Gatos Recreation Department and pay the After-School Sports Fee (a link will be sent to you in the Welcome Letter). AGAIN, this feature is only to be used AFTER you are notified that your child has made the team (for NO CUT teams, this link will be open to you after you have completed the JOT FORM). If you have difficulty with registration online, please call Los Gatos Recreation directly, 408-354-8700.  The Recreation Department will make the registration live once we are ready.  Highly popular, non-cut sports like Cross Country, we will notify parents a few days in advance when the link will open.  Those spots fill up fast and we do have a capacity for Cross Country/Track.
