Required Forms

The below mandatory documents must be filled out prior to showing up to tryouts/practice. 
No papers need to be delivered to the Athletic Department.  I will receive a digital confirmation once you have completed the forms.  
*If your child makes the team, a separate registration link will be provided but once teams are formed.
In other words, don't do #2 just yet.  
 
You must register online with the Los Gatos Recreation Department and pay the after school sports fee. AGAIN, this feature is only to be used AFTER you are notified that your child has made the team.   If you have difficulty with registration online, please call Los Gatos Recreation directly, 408-354-8700.  The Recreation Department will make the registration live once we are ready.  Highly popular, non-cut sports like Cross Country, we will notify parents a few days in advance when the link will open.  Those spots fill up fast and we do have a capacity for Cross Country/Track.

Anyone who plans to drive any students other than their own, MUST have this form on file in the boys PE office with Mr. Mercer.
