Forms & Deadlines

For any service performed, please complete the Community Service Form. 

 

For any group service projects, each student must complete a separate Community Service Form.

 

For multi-day service projects (e.g, helping a teacher every Monday), please note the occurrence on the Community Service Form.

 

Student's  - You may optionally keep track of all your hours on this Personal Service Log.

 

Be sure to submit approved community service forms within 2 months of the work performed in order to receive credit.  If students suspect an error, they should contact Mrs. Tiscareño-Kennedy or the HSC Fisher Service Chair immediately.

 

Turn in approved forms by May 13, 2026 to get credit towards recognition at promotion.
