Forms & Deadlines

For any service performed, please complete the Community Service Form. 

 

For any group service projects, each student must complete a separate Community Service Form.

 

For multi-day service projects (e.g, helping a teacher every Monday), please note the occurrence on the Community Service Form.

 

Student's  - You may optionally keep track of all your hours on this Personal Service Log.

 

Be sure to submit approved community service forms within 2 months of the work performed in order to receive credit. Coordinators will process the forms and confirm with the student’s parent on a quarterly basis. If students suspect an error, they should contact their Grade Level Coordinator immediately.

 

Turn in approved forms by May 10, 2024 to get credit towards recognition at promotion.

 

Each grade has a dedicated Community Service Coordinator.  These coordinators are responsible for processing submitted Community Service Forms and tracking total hours for participating students. If you have questions, please reach out directly to your student’s Grade Level Coordinator.
